ENROLLMENT FORM

I hereby enroll myself or my child as a student of BackBeat City. I agree to pay the tuition for all reserved classes reserved, whether utilized or not. Participation in BackBeat City productions is optional, but students in performing groups are encouraged to participate and private lesson students are welcome to audition for an opportunity to showcase their skills. If my student participates in a production, I agree to pay for any costumes or accessories, as well as any production fees, which may be in the form of ticket sales.

I understand that group class tuition is due for the entire school year and is payable in semester installments. The first payment is due upon registration and the second payment is due 30 days later. I understand that if I am on a payment plan and my student chooses to stop participating in their BackBeat City program, we are still responsible for all future installments. A $10.00 late fee will be charged to my account for every month that my payment is late. In addition, any check returned to the school unpaid by the bank will be subject to a $25.00 service charge. I understand that no installment statements will be mailed and that I am able to contact the office to check my account balance.

I understand that there are no refunds of registration, tuition fees or costumes paid. I understand that costumes will not be delivered if there is a past due balance for costumes or tuition fees. I understand that my student will not be allowed to participate in the end of the year recital unless all tuition, costume payments or any other charges to my account are paid prior to dress rehearsal. I agree that placement of Enrollee in BackBeat City programs/classes is at the sole discretion of BackBeat City Staff.

In addition to the above, I have read these and the other school policies contained in the school brochure and I understand and agree to abide by them while my student is enrolled at BackBeat City.

WEBSITE/ADVERTISING IMAGE RELEASE

I hereby confer on BackBeat City the absolute and irrevocable right and permission with respect to the photographs and videos that may be taken of my minor child at various BackBeat City events in which he/she may be included and:

• To copyright the same in BackBeat City’s name or any other name that it may select;
• To use, re-use, publish and re-publish the same in whole or in part, separately or in conjunction with other
  photographs, in any medium now or hereafter known, and for any purpose whatsoever, including (but not by
  way of limitation) website, promotion and advertising, and;
• To use my name or my child’s name in connection therewith.

I hereby release and discharge BackBeat City from all and any claims and demands ensuing from or in connection with the use of the photographs and videos, including any and all claims for libel and invasion of privacy. This authorization and release shall inure to the benefit of the legal representatives, licensees and assigns of the photographer as well as the person(s) or entity for whom he/she took the photographs.

RELEASE OF ALL CLAIMS

In consideration of the permission granted to Enrollee to enroll and participate as a student at BackBeat City I hereby release for myself, Enrollee and any one claiming through Enrollee and agree to hold harmless and fully indemnify BackBeat City its Members, Employees, Agents, Instructors and affiliated persons from and against any costs, expenses, claims, charges, demands, liability, harm or damages, including reasonable attorneys fees, which may result in any way from Enrollee’s participation as a BackBeat City student.

I fully understand that Enrollee assumes all the risks in connection with enrolling and participating in the activities associated with being a BackBeat City student. I understand that there are risks of injury that are attached to some activities and that BackBeat City shall not be responsible for any damage, including personal injury, that results therefrom.

I, the undersigned, have read this Enrollment Form and understand the release, hold harmless and indemnification aspects contained herein. I execute it voluntarily and with full knowledge of its significance. I hereby certify that I am the parent or legal guardian of this student and have full authority to execute this agreement on his/her behalf.
